Tips on what information to put on Bracelet Tags 

SUGGESTED DOs
  • Make sure you list the most critical information on the tag.
  • Separate lists of medical conditions with commas, e.g.: Allergic to penicillin, peanuts & Beta-Blockers
  • If your list of medical conditions, allergies, or medicines cannot fit on one side, due to the number of characters, please leave the bottom line to say: -see over-
  • If your complete medical information will not fit on the tag, leave the bottom line of the back side to say: see wallet card.

SUGGESTED DON'Ts
  • If you live alone, do not use your own phone number (if you choose to list one) as the contact number listed on the bracelet.
  • Because they are subject to change, do not include dosage information.
